Adaptive & Specialized Rec Intake Form

For New Adaptive & Specialized participants only: To ensure you get connected to programs that are relevant to your/the participant's needs, please take a moment to fill out this intake form. Adaptive & Specialized staff will review forms and reach out to discuss upcoming activities that may be of interest.

Adaptive & Specialized Activity Levels

A&S Level I

Level I activities are highly supported and structured and intended for participants who require any of the following: beginner level introduction to the subject matter, fully enclosed/secure spaces, slower paced instruction or physical activities, lots of breaks between physical or mental demands, supported transitions with lots of choices, and decreased demands to help facilitate participant enjoyment and success. Participants in Level I activities must still be able to not present a safety risk to themselves or others and be able to follow staff directions with reasonable support. Participants with specific needs are encouraged to submit an Accommodation Request form at least 2 weeks prior to the start of the program.

A&S Level II

Level II activities are well supported but have higher participant : staffing ratios than Level I activities. Level II activities include those that may have slightly higher physical and/or attention and self regulation demands than Level I activities, and may not be fully enclosed or have access to extra supports such as quiet rooms and/or different alternative activity choices. Participants in Level II activities should be able to follow staff directions reliably and with minimal support, as well as maintain mental or physical stamina for longer periods of time (20 minutes or more) with fewer built in breaks. Participants with specific needs are encouraged to submit an Accommodation Request form at least 2 weeks prior to the start of the program. Those requiring more support to meet these expectations may request a caregiver to join them in an Accommodation Request.

A&S Level III

Level III activities are best suited for participants who can fully self-regulate emotions, self-advocate for needs, and fully follow staff directions independently with minimal or no reminders or support. While the activity may still be physically accessible, participants may be expected to be able to engage in activities for longer periods of time and/or at a faster pace than Level I & Level II activities.Level III activities are not always fully enclosed, may require mental or physical attention and stamina in excess of 40 minutes at a time without built in breaks, and have the highest participant to staff ratio of our programs. Participants with specific needs are encouraged to submit an Accommodation Request form at least 2 weeks prior to the start of the program.

Volunteer Opportunities

We can always use a helping hand! Apply for opportunities in adaptive and specialized programming, such as coaching for Unified Sports Leagues, register as a caregiver, or apply as a general volunteer.
